My memory has been affected but I use reminders on my phone to prompt me to take my medication etc but can be easily distracted so may need a reminder from you.
I can access the community but would like to do more and will need your support to get out and about.
I will need to be encouraged to complete certain things, like personal grooming and dressing and domestic tasks within the home.
I have a very loving and supportive family; my partner and my two children are a constant support to me, but they need a break from time to time.
