We’re changing how our training quizzes work. From Monday 12 October, all our training quizzes will be on MS Forms.
What you need to do
If you have an existing quiz on SurveyMonkey, please complete it by 11.59pm on Friday 16 October.
After this date, our SurveyMonkey account will close. Any quizzes completed on SurveyMonkey after the closure date, and their certificates, may not be actioned.
Bought your workbook recently?
If you ordered or bought a workbook on or after 9 September 2026 and still need to complete the quiz after 16 October, email us at training@independentlives.org. We’ll send you a new link to the MS Forms quiz.
