Drivers licence required: Yes

Hours per week:

18 hours a week from the following shifts Monday, Tuesday, Thursday Saturday and Sunday mornings for 4 hours and up to 2 hours in the evenings. Actual shift times by mutual agreement.

Hourly pay rate:

£13.00 rising to £15.00ph once fully trained

I am a young woman in my thirties and live with my two indoor cats.

I like to be independent but currently need a lot of support around my flat and with personal care including health care tasks due to several health issues after a rough couple of years.

The job will involve helping with stoma care, tube feeding, medications, domestic tasks, accessing the community and most importantly helping me avoid hospital admissions.

As a senior PA, duties will also include being a role model for PAs to follow, demonstrating exemplary practice. Working with other professions to ensure care and social needs are supported in a person-centred way, recognising individuality.

Work with me to ensure all care documentation is accurate, relevant and reflects directives from professionals involved in my care

Completion and maintenance of clear and accurate care records, care plans and risk assessments and communication systems, ensuring these are used effectively

Duties include:

All rubbish and recycling out
Help organise paperwork
Check for out of date food.
Putting out clinical waste once a week
Making up hot water bottles
Changing the bed
Make up fresh St Mark's solution daily
Prepare medication for JEJ tube
Run me a bath & wash hair when needed
Stoma care
Set up and connect me to my feed St Mark's solution.
Looking for signs of low blood sugar and testing to see if I'm having a hypo, then administering medication if needed
Help getting to appointments & being an advocate
Collecting medications from pharmacy
Accessing the community Sunday shift will include accompanying me to wheelchair rugby.
Support with employment responsibilities, seeking advice from Independent Lives as required
Plan and manage staff rotas to ensure there is sufficient cover to meet needs
Plan, implement and monitor PA inductions, providing additional training and support as required
Arrange all mandatory training, booking online and ordering Workbooks through Independent Lives and allocating time for completion
Liaise with Case Manager at CHC to book clinical training
Maintain training records, ensuring training is up-to-date and relevant to meet care needs
Undertake and maintain records of 6-8 weekly supervisions with all PA's, discussing any issues with employer
Work alongside other PAs to meet personal and social care needs.
Monitor and manage care files and work with employer and other professionals and PAs to ensure that these are updated and reflective of individual needs.
Liaise with other professions when necessary to ensure that all health and social care needs are meet.
